Income Limit
At or below $47,880 a year for a household of one, $64,920 for two, $81,960 for three and $99,000 for four, plus $17,040 for each extra person (GSK's "maximum annual gross income", which is 300% of the 2026 poverty guideline). Higher limits apply in Alaska ($59,850 for one) and Hawaii ($55,080 for one); Puerto Rico has its own table ($28,800 for one).
Insurance Requirements
There are two routes. No prescription coverage: you must have no prescription drug benefits through any insurer or programme, and no Medicaid, VA, DOD or TRICARE drug coverage. Medicare Part D: you must have a Medicare drug plan and have paid a total of $600 for prescriptions in the current calendar year. Either way you must live in the United States or Puerto Rico. Patients with commercial insurance use the Trelegy savings card instead.

What changed on this page. On 23 August this page replaced a 350% limit with 400% "reduced from 500%" and a Medicare rule about plans that supply Trelegy free. On 4 October we checked GSK's foundation pages: the limit is $47,880 for one person (300% of the poverty guideline), neither 400% nor 500% appears, and the Medicare route requires a Part D plan plus $600 paid for prescriptions this calendar year. Both are corrected above, along with the apply link (which pointed at the co-pay card) and the price (GSK lists $697.73 for 30 days, not about $550).
